Anti-Wrinkle Injections: What You Really Need to Know

If you've been considering anti-wrinkle injections — or you've had them before and want to understand them better — you've probably encountered a lot of conflicting information. Frozen faces. Celebrity horror stories. Miracle transformations. The internet is full of opinions, but very short on genuine medical insight.

As a GMC registered doctor with a background in emergency medicine and a specialist interest in medical aesthetics, I want to give you the honest, evidence-based picture. Not a sales pitch — a clinician's view of what botulinum toxin actually is, what it can and can't do, and what separates a safe, effective treatment from a risky one.

The Biggest Myth: 'It Has to Freeze Your Face to Work'

This is, without question, the misconception I hear most often — and the one that frustrates me most as a doctor.

The idea that anti-wrinkle injections must leave you expressionless to be effective is simply not true. When administered correctly, with sound anatomical knowledge and good technique, botulinum toxin relaxes targeted muscles and smoothes the overlying skin — without erasing your natural expression.

The frozen, 'done' look that people fear is almost always the result of poor technique, incorrect dosing, or insufficient understanding of facial anatomy. It is not an inevitable outcome of the treatment itself. Done well, anti-wrinkle injections should be almost imperceptible. People should notice that you look refreshed — not that you've had something done.

How Does Botulinum Toxin Actually Work?

Botulinum toxin works by blocking the receptors on muscles that normally respond to signals from the nerves. By interrupting this communication, it allows the muscle to relax — and when the muscle is relaxed, the lines that form on movement of that muscle are significantly reduced.

It is not permanent. Results typically last an average of three to six months, though this varies considerably between individuals and is influenced by factors such as your metabolism, the area treated and the dosage used.

One fact that genuinely surprises most people: when used in the skin itself rather than the underlying muscle, botulinum toxin is highly effective at reducing sweat production. This leads to clearer pores and, in some patients, fewer breakouts — something we'll come back to in more detail below.

What Our Consultation Process Looks Like

Before any treatment at Revitalise Aesthetics, every patient undergoes a thorough, one-on-one doctor-led consultation with a GMC registered practitioner. This is not a formality — it is the foundation of safe, effective treatment.

We begin by reviewing your medical history. Certain conditions and medications can make anti-wrinkle injections unsafe or unsuitable and identifying these before treatment is a non-negotiable part of our process. We also confirm that patients are not pregnant or breastfeeding, as treatment is contraindicated in both cases.

From there, we conduct a detailed assessment of your facial anatomy. This is where medical training makes a meaningful difference. Understanding the precise location of muscles, how they interact and how individual anatomy varies between patients is essential for achieving symmetrical, natural-looking results — and for avoiding unintended effects in surrounding areas.

We only ever use safe, legitimate, pharmaceutical-grade products. And every patient is reviewed two weeks after their appointment, giving us the opportunity to assess results and make any dosing adjustments that may be needed.

Who Is a Good Candidate for Anti-Wrinkle Injections?

A suitable candidate for anti-wrinkle treatment is generally someone who:

  • Is over 18 years of age
  • Is not pregnant or breastfeeding
  • Has no medical contraindications to the treatment
  • Has a clear understanding of both the benefits and the risks
  • Is able to follow aftercare instructions
  • Is willing to attend a review appointment to assess the outcome

Botulinum toxin works on dynamic lines — lines that appear with movement. Once lines are deeply set and visible at rest, it is not the ideal treatment of choice, because those static lines are caused by collagen loss and skin laxity rather than muscle movement alone.

Used earlier, however, anti-wrinkle injections can prevent those dynamic lines from ever becoming static. They essentially stop the repetitive muscle movements that, over time, etch permanent lines into the skin. This makes them an excellent preventative tool.

That said, the decision of when to start is entirely personal. Only the individual can know when they feel ready. Our role is purely to advise from a medical standpoint on whether treatment is appropriate — there is no single 'right' time to begin.

What to Expect After Treatment

Setting realistic expectations is something we prioritise at every consultation. Here is what a typical post-treatment experience looks like:

  • Effects begin to become noticeable 3 to 5 days after treatment
  • Maximum effect is usually reached at 10 to 14 days post-treatment
  • Some initial asymmetry is normal and usually resolves naturally — any remaining unevenness can be addressed at the two-week review

One risk we always counsel patients on is the potential for diffusion of the product into surrounding muscles, which can temporarily affect movement in adjacent areas. We minimise this risk significantly through a thorough anatomical assessment and precise injection placement.

Aftercare Instructions

  • Remain upright for several hours following treatment
  • Avoid touching or rubbing the treated area — this reduces the risk of product migration
  • Avoid alcohol and strenuous exercise for 24 to 48 hours to minimise bruising risk
